Send the negative films, slides,
or photos by postal, UPS, or
FedEx. The balance amount is
due upon e-mail notification
of your order completion. All
information entered below is
private and confidential. Minimun

| PREPARATION |
| 1. |
Arrange
your photo negatives in the order you wish
to have them appear on the DVD video. |
| 2. |
Group
together all photo negatives that you want
them to be together under one chapter point
in DVD. Put them in an envelope and mark
the sequence number and the titles you wish
to have it on the menu buttons. Maximum
15 characters per chaper buttons. |

NOTE:
You can have a maximum of 10 chapter points
per DVD disc. |

Place
them in a plastic bag and sealed it (i.e.
ziplock bags). |
| 4. |
Please include a printed copy of this order
form. |

the negative transfer order is completed,
you will be contacted by email or by phone.
You pay us the balance and we will ship
you the DVD right away. |

whole transfer service process will take
about a week. Your DVD will arrive in 5-7
business days from the date you submit the
order. |

liability for any loss, damage, or
delay to tapes or other provided
material during the transfer process
or while in transit will be limited
to the replacement cost of a new
photo negative films of similar
size and or format. Except for such
replacement, CMT-BIZ shall not be
liable for any loss or damage, direct,
consequential or incidental by customers
use of CMT-BIZ's service.
To
the best of my knowledge the
video material I am submitting
to CMT-BIZ for transfer and or
conversion is not protected by
copyright.
